COURT FORMS AND RULES
The use of current mandatory
forms is required in any family law action. Family Law forms
packets may be purchased at the Court Facilitator Office.
Please contact the Court Facilitator at 675-5415 to make
arrangements to pick up a packet of forms in family law matters.

- Domestic violence, sexual assault and vulnerable adult protection
forms can be obtained from the Superior Court Clerk's office.

- Samples of local court forms can be viewed in the Local
Court Rules. Do not use the forms in the Local Court Rules. They do not comply with formatting requirements. _
